# Adding Forms to Services

Attach one or more existing forms to a service so they are included with its appointments.

This page shows you how to attach one or more existing forms to a service.

1. Select the service you want to add a form to.

## What happens next

* Any appointments booked for that service now include the forms.
* If the service is booked online, its forms automatically show at the end of the online booking process.
* Emails can include forms using the \[FORMS] tag in the email reminder and online booking templates. See [sending forms](/simplespa-documentation/daily-operations/forms/sending-a-form.md) for more details.
